{Index} does not work on some pages unless I remove the last few images on that page. How do I go about getting it to work correctly?
All pages have the same characteristics (header 2, paragraphs, images, etc).
The page has a LOT of html. Wit a LOT of characters.
So my guess is that the php is running into memory limits when trying to search/replace things in the content (using Regular Expressions).
See if you can solve it by increasing PHP memory limits.

Hi, I have the same issue. I removed the <p> tags inserted by the editor, and checked the hosting settings: PHP 7.4.22 and memory limit 1024M. All of this didn't help. You can see an example here:

You can use the 'Confidential information' button in the forum editor to hide sensitive information.
Well, this is indeed also down to the php memory limit.
The jrModuleSlider thing you have on those pages, generates an enormous amount of html output. And the php chokes on that trying to find and replace in it.
I've looked at the code you're talking about, but I don't find anything abnormal about it. I also asked the developer about the component that generates it (Jreviews), and he too replied that it looks like normal html.
To avoid headaches, in the url below I have completely turned off the output associated with jrModuleSlider, and Quick Index still does not appear.

To lighten the page further, I also tried disabling some features like the map and gallery, but that didn't help either.
It looks like Quick Index is actually replacing the index tag with the full index in the article text correctly. But is then overwritten (reverted back to what it was) in the next stage by something.
You have a LOT of extensions and frameworks installed. So it is pretty much a labyrinth to see what would be causing that.
I recommend you try and reproduce the issue on a clean installation with the Joomla default template and no other 3rd party extensions installed.
If you can't see the issue there, try adding stuff you have installed to see when it breaks... then we have an idea what is causing it or conflicting with Quick Index...
I am happy to confirm that the update resolves the reported issue: I updated the plugin and after clearing the cache QuickIndex it reappeared from where it was missing.
I have the same problem - {index} works fine on some pages, but not on the others.
Today I upgraded to 3.1.0 Pro and that solved problem in this article -

On this page Sliders are also used to hide contents.
On some other articles, where also Articles Anywhere is used I have one long article in which {index} is not working, and two other articles in the same series, in which {index} is working.
